TSK ve Emniyet g\u00fc\u00e7lerinin g\u00fcvenlik ama\u00e7l\u0131 u\u00e7u\u015flar\u0131 d\u0131\u015f\u0131nda, yolcu ve kargo ta\u015f\u0131mac\u0131l\u0131\u011f\u0131, zirai m\u00fccadele, yang\u0131n s\u00f6nd\u00fcrme, hava taksi, hava ambulans\u0131, hobi ve sportif ama\u00e7l\u0131 u\u00e7u\u015flar hava saham\u0131zda bir yo\u011funluk yaratmakta, ister istemez kaza veya risk oranlar\u0131 da artmaktad\u0131r. \u00dclkemizde THY\u2019nin 1956 y\u0131l\u0131nda ta\u015f\u0131d\u0131\u011f\u0131 yolcu say\u0131s\u0131 218 bin iken, 22 y\u0131l sonra (1978) di\u011fer \u015firketlerle birlikte, on kat artarak 2.2 milyona ula\u015fm\u0131\u015f, 1987\u2019de 9 milyon, 1997\u2019de 35 milyon, 2006\u2019da 61 milyon olmu\u015ftur; d\u00fcnyada ise bu rakam y\u0131lda 4 milyardan fazlad\u0131r.<\/p>\n<p>Bu kadar \u00e7ok ki\u015fiyi ilgilendiren ve t\u0131bbi deste\u011fin \u015fart oldu\u011fu bir alanda \u00f6zel t\u0131bbi \u00fcnitelerin bulunmas\u0131 gerekir. Her ya\u015f, cins ve meslek grubundan insanlara hizmet verebilen genel hastaneler a\u00e7man\u0131n kolayl\u0131\u011f\u0131 varken, havac\u0131lar i\u00e7in ayr\u0131 hastaneler kurmak, buralarda konular\u0131nda uzmanla\u015fm\u0131\u015f u\u00e7u\u015f doktorlar\u0131 \u00e7al\u0131\u015ft\u0131rmak ve \u00f6zel t\u0131bbi ara\u015ft\u0131rmalar yapmak gereksiz ve l\u00fcks say\u0131lmamal\u0131d\u0131r.<\/p>\n<p><span style=\"color: red;\">Havac\u0131l\u0131k T\u0131bb\u0131n\u0131n \u0130lgi ve U\u011fra\u015f Alan\u0131<\/span><\/p>\n<p>Havac\u0131l\u0131k t\u0131bb\u0131, u\u00e7a\u011fa binerek y\u00fcksek h\u0131z, irtifa, d\u00fc\u015f\u00fck bas\u0131n\u00e7, radyasyon, G kuvvetleri ve 3 d\u00fczlemde hareketlere maruz kalan pilot, m\u00fcrettebat ve yolcularda ortaya \u00e7\u0131kan t\u0131bbi problemlerle ilgilenen, bunlara \u00e7\u00f6z\u00fcm veya \u00f6nlemler geli\u015ftiren bir t\u0131p dal\u0131d\u0131r. Yolcu veya m\u00fcrettebat olarak u\u00e7an insanda g\u00f6r\u00fclebilen rahats\u0131zl\u0131klardan belli ba\u015fl\u0131 olanlar\u0131 \u015funlard\u0131r : hipoksi, vertigo, u\u00e7ak tutmas\u0131, dekompresyon hastal\u0131klar\u0131 (barotitis, bend, choke, kar\u0131n gazlar\u0131 geni\u015flemesi), g\u00f6rsel ill\u00fczyonlar,jet-lag, G kuvvetlerinin neden oldu\u011fu bozukluklar, u\u00e7u\u015f korkular\u0131, vs. Hava &#8211; uzay hekimli\u011fi uzmanlar\u0131 ve u\u00e7u\u015f doktorlar\u0131, bu ve benzeri rahats\u0131zl\u0131klar\u0131n tan\u0131 ve tedavilerinden sorumludurlar. D\u00fcnya askeri havac\u0131l\u0131\u011f\u0131nda u\u00e7u\u015f doktorlar\u0131n\u0131n etkin rol almalar\u0131n\u0131n tarihi 1916, T\u00fcrk Hava Kuvvetlerinde ise 1929 y\u0131l\u0131na dayanmaktad\u0131r.<\/p>\n<p><span style=\"color: red;\">U\u00e7u\u015f kazalar\u0131nda insan fakt\u00f6r\u00fc<\/span><\/p>\n<p>U\u00e7ak kazalar\u0131, 4 M form\u00fcl\u00fc ile ( Machine, Medium, Mission, Man) a\u00e7\u0131klan\u0131r. Buradaki en \u00f6nemli M, insand\u0131r. &#8220;U\u00e7u\u015f kazas\u0131n\u0131 yapan insan\u0131n pilot oldu\u011fu&#8221; eski d\u00fc\u015f\u00fcncesine kar\u015f\u0131 bug\u00fcn, sistem i\u00e7indeki di\u011fer insanlar\u0131n da hatalar\u0131, ki\u015fisel zaaf ve yetersizlikleri, kazalarda birinci derecede rol oynama potansiyelinde kabul edilmektedir. Bu nedenle pilot kadar di\u011fer m\u00fcrettebat, bak\u0131mc\u0131, m\u00fchendis, hava trafik kontrol\u00f6r\u00fc, vs. u\u00e7u\u015f g\u00fcvenli\u011finde insan unsurunun elemanlar\u0131 say\u0131lmaktad\u0131r. Ba\u015fka \u00fclkelerdeki durum nedir ?<\/span><\/p>\n<p>\u00dclkemizin de kurallar\u0131na uymakla y\u00fck\u00fcml\u00fc oldu\u011fu Uluslararas\u0131 Sivil Havac\u0131l\u0131k Birli\u011finin (ICAO) 1985 tarihli pilot lisans\u0131 y\u00f6nergesinde (B\u00f6l\u00fcm 1, madde 2.4.4) pilot muayenelerinin havac\u0131l\u0131k t\u0131bb\u0131 e\u011fitimi alm\u0131\u015f doktorlar taraf\u0131ndan yap\u0131laca\u011f\u0131 h\u00fckm\u00fc vard\u0131r.<\/p>\n<p>1997 tarihli JAR-FCL 3 (Joint Aviation Requirements- Flight Crew Licensing) de, pilotaj muayenelerinin en az 60 saatlik bir havac\u0131l\u0131k t\u0131bb\u0131 kursundan ge\u00e7mi\u015f doktorlar (authorised medical examiner) taraf\u0131ndan yap\u0131lmas\u0131n\u0131 gerektiren maddeler i\u00e7ermektedir.<\/p>\n<p>ABD.\u2019de 700 bin havac\u0131n\u0131n \u00fcye oldu\u011fu Federal Aviation Administration (FAA) 1929 y\u0131l\u0131ndan beri havac\u0131l\u0131\u011f\u0131n her alan\u0131nda (u\u00e7u\u015f g\u00fcvenli\u011fi, s\u00f6rtifikasyon, e\u011fitim, ara\u015ft\u0131rma, vs.) \u00e7al\u0131\u015fmalar yapmakta olup, b\u00fcnyesinde 300 u\u00e7u\u015f doktoru ve 5.600 u\u00e7u\u015f muayenesi yapmaya yetkili hekim (aeromedical examiner) bulunmaktad\u0131r. Bunlar\u0131n b\u00fcy\u00fck b\u00f6l\u00fcm\u00fc ABD. i\u00e7indedir ancak, her \u00fclkede de bu ehliyete sahip hekimlerle anla\u015fmalar yap\u0131lm\u0131\u015ft\u0131r. FAA., ba\u011fl\u0131s\u0131 olan sivil havac\u0131l\u0131k t\u0131bb\u0131 enstit\u00fcs\u00fc (CAMI) arac\u0131l\u0131\u011f\u0131yla u\u00e7u\u015f muayenesi yapmaya yetkili doktorlar\u0131n (aviation medical examiners) 4.5 g\u00fcn s\u00fcren bir kurstan ge\u00e7melerini, her 3 y\u0131lda bir 2.5 g\u00fcnl\u00fck tazeleme kursu almalar\u0131n\u0131 istemektedir. Lufthansa\u2019da benzer yetki ve sorumluluklar\u0131 olan hekim say\u0131s\u0131 16 ; Swissair\u2019de 10\u2019dur. Singapur\u2019un bir havac\u0131l\u0131k akademisi bulunmaktad\u0131r. Kanada, Avustralya, Yeni Zelanda, \u0130svi\u00e7re, \u0130srail , Yunanistan, Polonya vs. \u00fclkelerde havac\u0131l\u0131k t\u0131bb\u0131 birlik ve dernekleri vard\u0131r. (\u0130nternet kanal\u0131yla ba\u015fka \u00fclkelerdeki uygulamalar da g\u00f6zden ge\u00e7irilebilinir.)<\/p>\n<p>&#8220;Havac\u0131l\u0131k t\u0131bb\u0131&#8221; veya &#8220;hava-uzay t\u0131bb\u0131&#8221;, esasen bir ihtisas dal\u0131d\u0131r. Aeromedical examiner stat\u00fcs\u00fcnden ba\u015fka, ABD.\u2019de 4 merkez taraf\u0131ndan (Hava Kuvvetleri Brooks-San Antonio\u2019da, Deniz K. Pensacola-Florida\u2019da, Kara K. Fort Rucker- Alabama\u2019da ve siviller Wright State University Dayton-Ohio\u2019da) 2-3 y\u0131l s\u00fcren ihtisas ile bu dalda \u00fcst d\u00fczeyde e\u011fitim verilmektedir.<\/p>\n<p><span style=\"color: red;\">T\u00fcrkiye\u2019deki durum ve havac\u0131l\u0131k t\u0131bb\u0131 uygulamalar\u0131<\/span><\/p>\n<p>D\u00fcnyada I. D\u00fcnya Sava\u015f\u0131ndan itibaren u\u00e7u\u015f doktorlar\u0131 yeti\u015ftirmeye ba\u015flanm\u0131\u015f, hatta bu sava\u015fta T\u00fcrk pilotlar\u0131 m\u00fcttefikimiz olan Almanlar\u0131n u\u00e7u\u015f hekimi olan Dr.Kauer\u2019den destek alm\u0131\u015flard\u0131r. T\u00fcrkiye&#8217; de ilk u\u00e7u\u015f doktorunun yurtd\u0131\u015f\u0131nda (Fransa) e\u011fitim almas\u0131 1929 y\u0131l\u0131na dayanmakta olup, 1948 y\u0131l\u0131ndan itibaren de TSK ihtiyac\u0131 i\u00e7in Eski\u015fehir Hava Hastanesi b\u00fcnyesindeki Merkezde u\u00e7u\u015f doktoru kurslar\u0131 a\u00e7\u0131lmaktad\u0131r. Daha ileri bir ad\u0131m olarak 1986 y\u0131l\u0131nda GATA&#8217; da Hava ve Uzay Hekimli\u011fi Ana Bilim Dal\u0131 kurulmu\u015f, 1990 y\u0131l\u0131nda ise Eski\u015fehir\u2019de u\u00e7ucular\u0131n u\u00e7u\u015fta kar\u015f\u0131la\u015ft\u0131klar\u0131 fizyolojik sorunlar\u0131 e\u011fitim amac\u0131yla, emniyetli laboratuar ko\u015fullar\u0131nda sim\u00fcle eden d\u00fcnyan\u0131n en modern cihazlar\u0131yla m\u00fccehhez bir Fizyolojik E\u011fitim Merkezi faaliyete ge\u00e7irilmi\u015ftir.<\/p>\n<p>T\u00fcrkiye\u2019de halen hava, kara, jandarma ve deniz kuvvetlerinin ihtiya\u00e7lar\u0131 i\u00e7in yeti\u015ftirilmi\u015f birka\u00e7 y\u00fcz u\u00e7u\u015f doktoru bulunmaktad\u0131r. Her y\u0131l yurti\u00e7i ve yurtd\u0131\u015f\u0131 kurslardan ge\u00e7erek, en az 30 yeni u\u00e7u\u015f doktoru da bu gruba kat\u0131lmaktad\u0131r. Bunlar\u0131n aralar\u0131nda AGARD\u2019\u0131n \u00e7al\u0131\u015fma gruplar\u0131na kat\u0131lan, uluslararas\u0131 bilimsel toplant\u0131larda bildiri sunan, akademik kariyer yapmakta olanlar da vard\u0131r. \u00dclkemizde sadece GATA T\u0131p Fak\u00fcltesi b\u00fcnyesinde bir hava ve uzay hekimli\u011fi ana bilim dal\u0131 vard\u0131r. Buna kar\u015f\u0131l\u0131k, hi\u00e7bir \u00fcniversitemizin b\u00f6ylesine havac\u0131l\u0131k t\u0131bb\u0131yla ilgili e\u011fitim veren, ara\u015ft\u0131rmalar yapan bir b\u00f6l\u00fcm\u00fc yoktur. Ula\u015ft\u0131rma Bakanl\u0131\u011f\u0131&#8217;nda 1, DHM\u0130, THY ve THK&#8217;da 2&#8217;\u015fer u\u00e7u\u015f doktoru g\u00f6revdedir.<\/p>\n<p>Bilindi\u011fi gibi, ICAO ve JAR ile bunlar\u0131n paralelinde THY\u2019nin haz\u0131rlad\u0131\u011f\u0131 FOM (Flight Operations Manual), u\u00e7u\u015f muayenelerinde u\u00e7u\u015f doktorlar\u0131na yetki ve sorumluluk vermektedir (B\u00f6l\u00fcm 6). Ancak 2004 y\u0131l\u0131 sonuna kadarki uygulamada bu muayeneler Cerrahpa\u015fa T\u0131p Fak\u00fcltesi gibi baz\u0131 \u00fcniversite hastanelerine, Erzurum, Adana, \u0130stinye gibi baz\u0131 Devlet Hastanelerine ve Alman Hastanesi, \u0130nternational Hosp. gibi baz\u0131 \u00f6zel hastanelere b\u0131rak\u0131lm\u0131\u015f durumda idi. T\u0131bbi yeterliliklerinden \u015f\u00fcphe etmeye hakk\u0131m\u0131z olmayan bu hastanelerdeki pilotaj muayeneleri, maalesef havac\u0131l\u0131k t\u0131bb\u0131 e\u011fitimi olmayan doktorlar taraf\u0131ndan yap\u0131lmakta idi. (Halbuki k\u00fc\u00e7\u00fck bir fabrikada \u00e7al\u0131\u015fmak isteyen bir doktordan bile, i\u015fyeri hekimli\u011fi kursu g\u00f6rd\u00fc\u011f\u00fcne dair sertifika istenmektedir.) Avrupa Birli\u011fi\u2019ne girmeye haz\u0131rland\u0131\u011f\u0131m\u0131z s\u00fcre\u00e7te, pilotaj muayenelerinin ICAO ve JAA\u2019n\u0131n \u00f6ng\u00f6rd\u00fc\u011f\u00fc bi\u00e7imde yap\u0131lmas\u0131, bunun i\u00e7in de Ula\u015ft\u0131rma Bakanl\u0131\u011f\u0131\u2019n\u0131n koordinesi ve THY \u00f6nc\u00fcl\u00fc\u011f\u00fc gerekirdi. Neyse ki, Derne\u011fimizin de uyar\u0131lar\u0131 sonucunda 2003-2005 d\u00f6neminde Ula\u015ft\u0131rma Bakanl\u0131\u011f\u0131 Hava Kuvvetleri ile protokol yapm\u0131\u015f ve 38 sivil u\u00e7u\u015f doktoru yeti\u015ftirilmi\u015f, bunlar\u0131n bir k\u0131sm\u0131 baz\u0131 hastanelerde g\u00f6reve ba\u015flam\u0131\u015flar, bu hekimlerin pilotaj muayenelerinde mutlaka bulunmas\u0131 ve imzalar\u0131n\u0131n al\u0131nmas\u0131 ko\u015fulu getirilmi\u015ftir. Sivil kesimde 4 binden fazla lisansl\u0131 sivil pilotun bulundu\u011fu \u00fclkemizde THY, THK gibi b\u00fcy\u00fck b\u00fct\u00e7eli kurulu\u015flar\u0131n da, \u00fcniversitelerimizin de konuya el atmam\u0131\u015f olu\u015flar\u0131, u\u00e7u\u015f g\u00fcvenli\u011fi bak\u0131m\u0131ndan bir zafiyet oldu\u011fu kadar, bilimsel sorumluluk anlam\u0131nda da bir eksikliktir. Yani T\u00fcrk sivil havac\u0131l\u0131\u011f\u0131nda u\u00e7u\u015f g\u00fcvenli\u011finde insan fakt\u00f6r\u00fcn\u00fc kontrol alt\u0131nda tutmak bak\u0131m\u0131ndan, havac\u0131l\u0131k t\u0131bb\u0131 ve u\u00e7u\u015f doktoru deste\u011finde bir zafiyet mevcuttur ve bunu i\u015fin i\u00e7inde olan herkes kabul etmektedir.<\/p>\n<p><span style=\"color: red;\">Problemin boyutlar\u0131<\/span><\/p>\n<p>A\u015fa\u011f\u0131daki sorular\u0131n cevaplar\u0131, problemin boyutlar\u0131 hakk\u0131nda bir kanaat verebilir :<\/p>\n<p>1. Sivil havac\u0131l\u0131kta pilot se\u00e7imi i\u00e7in yerle\u015fik bilimsel usuller, y\u00f6ntemler var m\u0131d\u0131r ?,varsa bug\u00fcne kadar ka\u00e7 pilot aday\u0131 t\u0131bbi nedenlerle elenmi\u015ftir ? Keza, yeti\u015fmi\u015f u\u00e7uculardan ka\u00e7\u0131 son 1 y\u0131lda t\u0131bbi nedenlerle ge\u00e7ici veya s\u00fcrekli olarak u\u00e7u\u015ftan al\u0131konmu\u015ftur ? (t\u0131bbi nedenlerle u\u00e7ucular\u0131n elenmesi veya u\u00e7u\u015ftan al\u0131konulmalar\u0131, u\u00e7u\u015f doktorlar\u0131n\u0131n kendilerini tatminleri i\u00e7in de\u011fil, mutlaka her pop\u00fclasyonda bir oranda g\u00f6r\u00fclen elveri\u015fsizliklerin atlanmad\u0131\u011f\u0131n\u0131n ve gerekli i\u015flemlerin yap\u0131ld\u0131\u011f\u0131n\u0131n g\u00f6stergesi olarak \u00f6nem ta\u015f\u0131r.)<\/p>\n<p>2. Zirai ila\u00e7lama, hava ambulans, hava taksi, yang\u0131n s\u00f6nd\u00fcrme gibi sekt\u00f6rlerde, ya da hobi, heyecan veya spor amac\u0131yla u\u00e7u\u015f yapanlar\u0131n t\u0131bbi uygunluklar\u0131 nas\u0131l ve kim taraf\u0131ndan denetlenmektedir ?<\/p>\n<p>3. U\u00e7u\u015f g\u00fcvenli\u011fini ilgilendiren t\u0131bbi konularda zaman zaman bilgi tazeleyici seminerler, kurslar ve uygulamal\u0131 sim\u00fclat\u00f6r e\u011fitimleri yap\u0131lmakta m\u0131d\u0131r ? B\u00fclten \u00e7\u0131karmak veya bro\u015f\u00fcr da\u011f\u0131tmakla sorunun \u00e7\u00f6z\u00fcld\u00fc\u011f\u00fcn\u00fc s\u00f6ylemek inand\u0131r\u0131c\u0131 m\u0131d\u0131r ?<\/p>\n<p>4. U\u00e7u\u015f s\u0131ras\u0131ndaki acil sa\u011fl\u0131k sorunlar\u0131na u\u00e7ak i\u00e7inde veya yerden m\u00fcdahale usulleri belirlenmi\u015f midir, bunu yapabilecek e\u011fitim alm\u0131\u015f personel var m\u0131d\u0131r ?<\/p>\n<p>5. U\u00e7ak yolcular\u0131ndan ciddi sa\u011fl\u0131k sorunlar\u0131 olanlardan hangilerinin u\u00e7mamas\u0131 veya hangi \u00f6nlemlerle u\u00e7abilece\u011fi konusunda bilgili ve yetkili ki\u015filer veya konuyu belirleyen \u00e7eklist, y\u00f6nerge, dok\u00fcman vs. var m\u0131d\u0131r ?<\/p>\n<p>6. U\u00e7u\u015f emniyetine y\u00f6nelik g\u00fcncel t\u0131bbi sorunlar \u00fczerinde k\u00fc\u00e7\u00fck \u00f6l\u00e7ekte de olsa herhangi bir bilimsel ara\u015ft\u0131rma yap\u0131lmakta m\u0131d\u0131r, cevab\u0131 aranan sorular var m\u0131d\u0131r ?, Havac\u0131l\u0131k t\u0131bb\u0131yla ilgili dergilere abone olunmu\u015f mudur, yurtd\u0131\u015f\u0131ndaki t\u0131bbi toplant\u0131lara personel g\u00f6nderilmekte midir, TSK\u2019n\u0131n ilgili \u00fcnitelerinden destek istenmi\u015f midir ?<\/p>\n<p>Bu sorular\u0131n cevaplar\u0131, bir durum tespiti yapmaya, y\u0131llard\u0131r devam eden ve kan\u0131ksanm\u0131\u015f bir zafiyeti belirlemeye ve uygun \u00f6nlemlerin neler oldu\u011funu anlamaya yarayabilir. Pilot ve m\u00fcrettebat\u0131n u\u00e7u\u015fa uygunlu\u011fuyla ilgili se\u00e7im a\u015famas\u0131ndan, yeti\u015fmi\u015f u\u00e7ucular\u0131n, hatta hava trafik kontrol\u00f6rlerinin periyodik muayenelerine, ve pilotlar\u0131n fizyolojik e\u011fitimlerine kadar, uygulanacak havac\u0131l\u0131k t\u0131bb\u0131 prosed\u00fcr\u00fcn\u00fcn ihmali, sonu\u00e7ta mutlaka u\u00e7u\u015f emniyetiyle ilgili bir zafiyet yarat\u0131r. Bilindi\u011fi gibi sa\u011fl\u0131k hizmetlerinde \u00fcst s\u0131n\u0131r yoktur, ne yap\u0131lsa daha fazlas\u0131 daha iyidir, ama bir ortalaman\u0131n da tutturulmu\u015f olmas\u0131 beklenir. Derne\u011fin, ba\u015fka \u00fclkelerdeki benzerleri gibi sivil havac\u0131l\u0131\u011f\u0131n gereksinim duydu\u011fu t\u0131bbi konularda (u\u00e7ucu se\u00e7imi, muayenesi, tedavisi, yolcular\u0131n sa\u011fl\u0131k sorunlar\u0131, vs.) destek olmak, u\u00e7ucular i\u00e7in seminerler tertiplemek, bilimsel ara\u015ft\u0131rmalar yapmak ve u\u00e7u\u015f doktoru yeti\u015ftirmek gibi ama\u00e7lar\u0131 vard\u0131r.
