Kaza raporlar\u0131 aksaman\u0131n \u00e7o\u011funlukla \u2018insanda\u2019 oldu\u011funu s\u00f6yler. U\u00e7u\u015f kazalar\u0131n\u0131n sebep analizlerinde insan fakt\u00f6r\u00fcn\u00fcn pay\u0131 1940 y\u0131l\u0131nda %70 olarak belirlenmi\u015fti. Uluslararas\u0131 Havac\u0131l\u0131k \u00d6rg\u00fct\u00fc\u2019n\u00fcn (ICAO) 2011 y\u0131l\u0131ndaki bir raporunda, (aradan ge\u00e7en 70 y\u0131lda m\u00fckemmeliyet s\u0131n\u0131r\u0131na varan teknik ilerlemelere kar\u015f\u0131n) kazalarda insan fakt\u00f6r\u00fc oran\u0131n\u0131n de\u011fi\u015fmedi\u011fi ve hatta biraz y\u00fckseldi\u011fi (%72-80) a\u00e7\u0131klanm\u0131\u015ft\u0131r (1,2). Bu oranlar sadece havac\u0131l\u0131k kazalar\u0131nda de\u011fil, i\u00e7inde insan bulunan t\u00fcm alanlarda (kara, deniz ve demiryolu ula\u015f\u0131m\u0131, end\u00fcstri, maden, n\u00fckleer, t\u0131p, spor ve ev kazalar\u0131nda) da ge\u00e7erlidir. \u00d6rne\u011fin T\u00fcrkiye\u2019de 1937 y\u0131l\u0131ndan bug\u00fcne kadar 100 bin yang\u0131n \u00e7\u0131kt\u0131\u011f\u0131; bunlardan sadece %3-5\u2019inde y\u0131ld\u0131r\u0131m gibi do\u011fal sebeplerin, %95\u2019den fazlas\u0131nda insan unsurunun bulundu\u011fu rapor edilmi\u015ftir\u2026<\/span><\/p>\n<p>Kazalar\u0131n \u00fc\u00e7 kayna\u011f\u0131; sistem hatas\u0131, \u00e7evresel unsurlar ve insan hatas\u0131d\u0131r. Sistem hatas\u0131, mekanik, teknik ve elektronik ar\u0131zalar\u0131 kapsar; ancak titiz incelemelerde bunlar\u0131n da insan hatas\u0131yla ba\u011flant\u0131s\u0131 oldu\u011fu g\u00f6r\u00fcl\u00fcr. Ayn\u0131 ba\u011flant\u0131 \u00e7evresel unsurlar i\u00e7in de ge\u00e7erlidir. Yani insana dair sorunlar kazalar\u0131n merkezinde yer al\u0131r\u2026<\/p>\n<p><strong><em><u>\u2018Vintage\u2019<\/u><\/em><u>\u00a0bir kavram: Pilot hatas\u0131<\/u><\/strong><\/p>\n<p>Havac\u0131l\u0131k kazalar\u0131nda son hatay\u0131 genellikle pilotlar yapt\u0131\u011f\u0131 i\u00e7in \u201c\u0130nsan Fakt\u00f6r\u00fc = Pilot Hatas\u0131\u201d form\u00fcl\u00fc akla yak\u0131n gelmi\u015ftir. Hava Kuvvetlerimizin 1990\u2019l\u0131 y\u0131llardaki bir slogan kampanyas\u0131nda \u00f6ne \u00e7\u0131kan c\u00fcmlelerden birisi, \u201cHatal\u0131 pilot yoktur, \u00e7\u00fcnk\u00fc \u00f6lm\u00fc\u015ft\u00fcr\u201d idi, ve bu c\u00fcmle bir \u00fcss\u00fcn giri\u015findeki tabel\u00e2ya yaz\u0131lm\u0131\u015ft\u0131. Verilen mesaj, \u2018\u00f6l\u00fcmc\u00fcl u\u00e7u\u015f kazalar\u0131 pilot hatas\u0131 sonucu olur,\u2019 ya da \u2018\u00f6lmek istemeyen pilot hata yapmamal\u0131d\u0131r\u2019 idi. Asl\u0131nda bu s\u00f6zler ger\u00e7e\u011fin sadece bir yan\u0131n\u0131 anlatmaktayd\u0131; baz\u0131 kazalar\u0131n ger\u00e7ekten de tamamen pilotun hatas\u0131ndan kaynakland\u0131\u011f\u0131 do\u011frudur, ama t\u00fcm kazalar\u0131 pilot hatalar\u0131na indirgemek do\u011fru mudur? \u00d6l\u00fcml\u00fc u\u00e7u\u015f kazalar\u0131n\u0131n sebep analizlerinden birinde bulunan sonu\u00e7lar \u2018hatal\u0131 pilot\u2019 \u00f6nyarg\u0131s\u0131n\u0131 yans\u0131tmaktad\u0131r:\u00a0<u>Mekanik-bak\u0131m<\/u>\u00a0%7.5,\u00a0<u>Di\u011fer unsurlar<\/u>\u00a0%9.6,\u00a0<u>Pilot<\/u>\u00a0%82.9 (3).<\/p>\n<p>Kan\u0131m\u0131zca pilot, u\u00e7u\u015fu ger\u00e7ekle\u015ftiren bir\u00e7ok insandan birisi, \u2018kokpitte oturan\u0131\u2019d\u0131r. Pilotu hata yapmaya zorlayan u\u00e7ak, bak\u0131m, teknik sistemler, hava trafik, y\u00f6netim, talimat, pl\u00e2nlama, organizasyon, ergonomi, t\u0131bbi ve psikolojik sorunlar gibi tasnif edilebilecek pek \u00e7ok ba\u015fka nedenler olabilir. \u00c7o\u011fu \u00f6rt\u00fcl\u00fc olan bu unsurlar\u0131n etkisiyle olu\u015fan bir kazada parmaklar\u0131n (tek sorumluymu\u015f gibi) pilotu i\u015faret etmesi haks\u0131zl\u0131kt\u0131r; geni\u015f perspektiften \u2018b\u00fcy\u00fck resme\u2019 bak\u0131lmal\u0131d\u0131r. Bir g\u00fcnah ke\u00e7isi bulup ona y\u00fcklenmek yerine, organizasyonel hatalar\u0131 ara\u015ft\u0131rmak en do\u011fru yakla\u015f\u0131md\u0131r. \u0130nsan fakt\u00f6r\u00fcn\u00fc\u00a0<em>\u2018vintage\u2019<\/em>\u00a0bir kavram olan \u2018pilot hatas\u0131yla\u2019 (4) e\u015fde\u011fer g\u00f6rme anlay\u0131\u015f\u0131, kaza nedeni olan di\u011fer unsurlar\u0131n g\u00f6zden ka\u00e7\u0131r\u0131lmas\u0131na veya gizlenmesine neden olur. Pilot hatas\u0131 terimi, son hatan\u0131n nerede oldu\u011funu g\u00f6sterir, ama pilotun neden hata yapt\u0131\u011f\u0131n\u0131 g\u00f6stermez&#8230;<\/p>\n<p><strong><u>Birbirini su\u00e7lama oyunu\u00a0<em>(Blame game)<\/em><\/u><\/strong><\/p>\n<p>\u0130nsanl\u0131k tarihi kadar eski ve k\u00f6k\u00fc genlerimizde olan bir oyundur. Bir kaza oldu\u011funda y\u00f6neticiler pilotlar\u0131, pilotlar hava trafik\u00e7ileri, vs. su\u00e7layarak kendilerini temize \u00e7\u0131karmaya \u00e7al\u0131\u015f\u0131r. Pilot \u00f6ld\u00fcyse onu (s\u00f6zde an\u0131lar\u0131na sayg\u0131 g\u00f6sterir gibi) do\u011frudan de\u011fil dolayl\u0131 yoldan su\u00e7lamak tercih edilir. Burada b\u00fct\u00fcn su\u00e7un pilota y\u00fcklenmesi oyunu oynanmakta, ba\u015fka unsurlar\u0131n \u00fcst\u00fc \u00f6rt\u00fclmektedir.<\/p>\n<p>Psikoloji profes\u00f6r\u00fc \u00dcst\u00fcn D\u00f6kmen\u2019in \u00e7ok at\u0131f alan bir yaz\u0131s\u0131 da \u2018g\u00fcnah ke\u00e7isi bulmak\u2019 tutumlar\u0131n\u0131n \u00f6rneklerini sunmaktad\u0131r: \u201c\u00c7ocu\u011fumuz d\u00fc\u015f\u00fcp kafas\u0131n\u0131 masaya \u00e7arp\u0131nca hemen masay\u0131 d\u00f6veriz; b\u00f6ylece \u00e7ocuk, masa orada durmasa kafas\u0131n\u0131 \u00e7arpmayaca\u011f\u0131n\u0131 san\u0131r ve b\u00fcy\u00fcd\u00fck\u00e7e yapt\u0131\u011f\u0131 her hatay\u0131 y\u00fckleyecek birini veya bir \u015feyi arar bulur\u2026 Balkan Harbinden kalma dandik raylardaki vagonlara 160 km h\u0131z yapt\u0131rd\u0131lar, ilk virajda sizlere \u00f6m\u00fcr; su\u00e7 kimin? Makinistin! Yollarda her g\u00fcn 20 insan\u0131m\u0131z heba oluyor; trafik canavar\u0131ndan. Dolar patlarsa enflasyon canavar\u0131ndan. Deli dana geliyor inekten, Evleri su bas\u0131yor ya\u011fmurdan. Ormanlar yan\u0131yor sigaradan. Band\u0131rma\u2019da gemi bat\u0131yor dalgadan. U\u00e7ak d\u00fc\u015f\u00fcyor rahmetli pilottan&#8230; Peki bu \u015fartlarda hayatta kalmay\u0131 nas\u0131l ba\u015far\u0131yoruz? Allah&#8217;tan&#8230;\u201d(5)<\/p>\n<p><strong><u>Ergonomi veya insan fakt\u00f6rleri disiplini<\/u><\/strong><\/p>\n<p>Amerikal\u0131lar insan fakt\u00f6r\u00fc, Avrupal\u0131lar ergonomi kavram\u0131n\u0131 tercih etmekte ise de bu iki kavram b\u00fcy\u00fck \u00f6l\u00e7\u00fcde e\u015f anlaml\u0131d\u0131r. Son y\u0131llarda ikisinin birle\u015ftirildi\u011fi\u00a0<em>\u2018Human Factors and Ergonomics Society\u00a0<\/em>(HFES)\u2019 kavram\u0131 kullan\u0131lmaktad\u0131r. Verimli, g\u00fcvenli, rahat ve etkili bir insan kullan\u0131m\u0131 sa\u011flamak amac\u0131yla ara\u00e7, gere\u00e7, makine, sistem, i\u015f, \u00e7al\u0131\u015fma ak\u0131\u015f\u0131-d\u00fczeni-\u00e7evresi tasarlamaktan sorumlu olan bu bilim, insan davran\u0131\u015f\u0131, yetenekleri, k\u0131s\u0131tl\u0131l\u0131klar\u0131 ve di\u011fer bilgileri ara\u015ft\u0131r\u0131r ve uygular (6).<\/p>\n<p>U\u00e7u\u015f kazas\u0131yla sonu\u00e7lanma ihtimali olan insan fakt\u00f6rleri \u00e7ok fazlad\u0131r; bir \u00e7al\u0131\u015fmada bunun say\u0131s\u0131 55 olarak g\u00f6sterilmi\u015ftir (7). B\u00fcy\u00fck b\u00f6l\u00fcm\u00fc insan\u0131n fizyolojik, bili\u015fsel ve psikolojik s\u00fcre\u00e7leriyle do\u011frudan ili\u015fkili olan insan hatalar\u0131n\u0131n kazalardaki pay\u0131n\u0131 azaltmak i\u00e7in \u00f6nce onlar\u0131n analizi, sonra u\u00e7u\u015f emniyeti zincirinde yer alan t\u00fcm payda\u015flarda fark\u0131ndal\u0131k yarat\u0131lmas\u0131 ve etkili \u00f6nlemlerin bu \u00e7er\u00e7evede olu\u015fturulmas\u0131 gerekir. Bu ba\u011flamda havac\u0131l\u0131k psikolojisi ve psikiyatrisinin rol\u00fc, sadece ruhsal patolojiler g\u00f6steren u\u00e7ucular\u0131n tan\u0131 ve tedavisiyle ilgilenmek de\u011fil, u\u00e7u\u015f kazalar\u0131na zemin haz\u0131rlayan psikolojik s\u00fcre\u00e7leri izlemek ve \u00f6nlemler geli\u015ftirmek olmal\u0131d\u0131r.<\/p>\n<p>ABD Federal Havac\u0131l\u0131k Dairesi Talimat\u0131nda (FAR. 8. B\u00f6l\u00fcm, 1. Madde) pilotun u\u00e7u\u015fa uygunluk halini bozan unsurlar, \u2018emniyetsizlik\u2019 anlam\u0131nda bir\u00a0<em>\u2018mnemonic\u2019<\/em>\u00a0ile (IMSAFE; I\u2019m Safe) sunulmu\u015ftur: Hastal\u0131k\u00a0<em>(Illness)<\/em>, \u0130la\u00e7 alm\u0131\u015f olmak\u00a0<em>(Medication)<\/em>, Stres, Alkoll\u00fc olmak, Yorgunluk\u00a0<em>(Fatigue),<\/em>\u00a0Duygusal nedenler\u00a0<em>(Emotion)<\/em>\u00a0(8,9). U\u00e7u\u015fu emniyetsiz hale getiren bu 6 unsurun hepsi de insan fakt\u00f6r\u00fc kavram\u0131n\u0131n, dolay\u0131s\u0131yla havac\u0131l\u0131k t\u0131bb\u0131n\u0131n (ve havac\u0131l\u0131k psikiyatrisinin) ilgi alan\u0131 i\u00e7indedir. Bu unsurlar, d\u00fc\u015fmeyecek \u015fekilde imal edilmi\u015f u\u00e7aklar\u0131, u\u00e7u\u015f ve yer ekiplerinin (insanlar\u0131n) ortakla\u015fa \u00e7abas\u0131yla d\u00fc\u015f\u00fcrebilir\u2026<\/p>\n<p><strong><u>Hayvan fakt\u00f6r\u00fc<\/u><\/strong><\/p>\n<p>U\u00e7an 8.600 ku\u015f t\u00fcr\u00fc vard\u0131r ve toplam say\u0131lar\u0131 100 milyardan fazlad\u0131r. Leylek, mart\u0131, bayku\u015f, yarasa, k\u0131rlang\u0131\u00e7, kartal, \u00f6rdek, g\u00fcvercin, albatros ve di\u011fer ku\u015flar ya da u\u00e7an binlerce cins b\u00f6ceklerin havada \u00e7arp\u0131\u015ft\u0131klar\u0131, sert indikleri, konamad\u0131klar\u0131, vb. gibi kazalar yok d\u00fczeyindedir. Belirli mevsimlerde s\u0131cak b\u00f6lgelere g\u00f6\u00e7 eden 5 milyar ku\u015f binlerce kilometre menzili y\u00f6nlerini kaybetmeden, hava ak\u0131mlar\u0131ndan enerji tasarrufu yaparak tamamlar. Bu hayvanlar hipoksi veya vertigo olmazlar, 6-10 bin metre irtifalardaki so\u011fu\u011fa dayanabilirler. Baz\u0131 u\u00e7an hayvanlar\u0131n gece g\u00f6r\u00fc\u015f yetenekleri vard\u0131r; yarasalar ise ultrasonik radar sistemleriyle avlar\u0131n\u0131 bulur, karanl\u0131kta bir yerlere \u00e7arpmadan s\u00fcratle u\u00e7abilirler. Amfibik ku\u015flar suya dalarak bal\u0131k yakalar. Sinek ku\u015flar\u0131 \u00e7i\u00e7ek \u00f6zlerini almak i\u00e7in havada sabit duracak kadar dengeli u\u00e7u\u015f yapabilir.
