Bu kan\u0131y\u0131 g\u00fc\u00e7lendiren en \u00f6nemli g\u00f6zlem, baz\u0131 pilotlar\u0131n u\u00e7u\u015fta ani geli\u015fen bel veya boyun a\u011fr\u0131lar\u0131 nedeniyle inkapasitasyon sonucu acil ini\u015f yapmak zorunda kalmalar\u0131d\u0131r. D\u00fcnyada en fazla i\u015f g\u00fcc\u00fc kayb\u0131na neden oldu\u011fu bilinen bel ve boyun a\u011fr\u0131lar\u0131n\u0131n uzun s\u00fcrmesi ve bu sorunlarla kar\u015f\u0131 kar\u015f\u0131ya kalan u\u00e7ucular\u0131n uzun s\u00fcreli u\u00e7u\u015ftan uzakla\u015fmas\u0131 nedeniyle bu durum daha da \u00f6nemli olmu\u015ftur.<\/p>\n<p style=\"text-align: justify;\">\u0130ntervertebral diskin (\u0130VD) dikey y\u00fcklenmesinde disk i\u00e7i bas\u0131n\u00e7 simetrik olarak da\u011f\u0131t\u0131l\u0131r. Ancak y\u00fcklenmenin bir tarafa olmas\u0131 durumunda disk i\u00e7indeki bas\u0131n\u00e7 asimetrik da\u011f\u0131l\u0131r. Bu da nukleus pulpozusun d\u00fc\u015f\u00fck bas\u0131nca do\u011fru yer de\u011fi\u015ftirmesine neden olur. Anulus fibrozis ise bunun tersine olarak y\u00fcksek bas\u0131nc\u0131n oldu\u011fu tarafta d\u0131\u015far\u0131 do\u011fru ta\u015far. Anulus fibrozisin d\u0131\u015far\u0131 ta\u015fmas\u0131, biti\u015fik omur cismi \u00fczerindeki kemik zar\u0131n\u0131n kemikten ayr\u0131lmas\u0131na ve burada da bir bo\u015fluk olu\u015fmas\u0131na neden olur. Daha sonra olu\u015fan bo\u015fluk i\u00e7inde kemiksi uzant\u0131lar (osteofit) geli\u015fir. Bu osteofitler genellikle omurgan\u0131n \u00f6n ve yan taraf\u0131nda olu\u015fur.<\/p>\n<p style=\"text-align: justify;\"><strong>Disk dejenerasyonu: <\/strong>Diskte ya\u015flanmayla birlikte ortaya \u00e7\u0131kan normal anatomik ve morfolojik de\u011fi\u015fikliklere \u0130VD dejenerasyonu denir. Dejenerasyon, disk hernisi olu\u015fumuna do\u011frudan neden olmaz fakat haz\u0131rlay\u0131c\u0131 bir fakt\u00f6rd\u00fcr. Omurgada geli\u015fen sorunlar ile klinik bulgular genellikle birbirine kar\u0131\u015f\u0131r ve bunlar\u0131 normal ya\u015flanma belirtilerinden ay\u0131rmak zordur. Soruna sadece radyografik g\u00f6r\u00fcnt\u00fclerle yakla\u015fmak hatad\u0131r. \u00c7\u00fcnk\u00fc radyografik boyun disk dejenerasyonu 50 ya\u015fa kadar olan insanlarda %25\u00ac\u00ac-50 oran\u0131nda vard\u0131r ve 65 ya\u015fa kadar %75-85 oran\u0131na y\u00fckselir. Kadavralar\u0131n servikal omurgalar\u0131nda yap\u0131lan bir \u00e7al\u0131\u015fmada C3-4 diskinin alt\u0131nda, a\u011f\u0131r de\u011fi\u015fiklikler olduk\u00e7a y\u00fcksek oranda bulunmu\u015ftur. Lomber b\u00f6lgede ise \u0130VD erkeklerde 20\u2019li ya\u015flarda g\u00f6r\u00fclmeye ba\u015flarken, kad\u0131nlarda 30\u2019lu ya\u015flardan sonra ortaya \u00e7\u0131kar. 50 ya\u015ftan sonra % 97 oran\u0131nda disk dejenerasyonu g\u00f6r\u00fclm\u00fc\u015ft\u00fcr. Bel omurlar\u0131nda en s\u0131k L3-4 ve L4-5 disklerinde dejenerasyon saptanm\u0131\u015ft\u0131r. Bu durum, zaten ya\u015flanma s\u00fcrecinde disklerde dejenerasyonun geli\u015fece\u011fini g\u00f6stermektedir. Fakat diskin \u00fczerine s\u0131k aral\u0131klarla olan y\u00fcklenme ve gev\u015femeler, disk dejenerasyonunu h\u0131zland\u0131ran bir fakt\u00f6r olarak g\u00f6r\u00fclmelidir. Bununla birlikte bu dejenerasyonun bel a\u011fr\u0131s\u0131n\u0131 izah edebilece\u011fi de d\u00fc\u015f\u00fcn\u00fclebilir.<\/p>\n<p style=\"text-align: justify;\"><strong>Pilotlar\u0131n bel-boyun a\u011fr\u0131lar\u0131:<\/strong> Jet pilotlar\u0131nda yap\u0131lan anket \u00e7al\u0131\u015fmalar\u0131nda, y\u00fcksek +Gz ile buna maruz kalma s\u00fcresinin artmas\u0131yla bel a\u011fr\u0131s\u0131 yak\u0131nmalar\u0131n\u0131n da artt\u0131\u011f\u0131; ya\u015f ile bel a\u011fr\u0131s\u0131 aras\u0131nda bir ilginin olmad\u0131\u011f\u0131 anla\u015f\u0131lm\u0131\u015ft\u0131r. Bu durum, a\u011f\u0131r sportif egzersizlerde oldu\u011fu gibi diskler \u00fczerine s\u0131k s\u0131k olan y\u00fcklenmelerin neden oldu\u011fu a\u011fr\u0131lara benzemektedir. Ayr\u0131ca +Gz maruziyetinin olmad\u0131\u011f\u0131 sivil hava yollar\u0131 u\u00e7ucular\u0131na uygulanan benzer bir anket \u00e7al\u0131\u015fmas\u0131nda da u\u00e7u\u015f s\u00fcresinin artmas\u0131 ile bel a\u011fr\u0131s\u0131 g\u00f6r\u00fclmesi aras\u0131nda do\u011fru ili\u015fki oldu\u011fu bildirilmi\u015ftir. Bu \u00e7al\u0131\u015fmalar, a\u011fr\u0131da post\u00fcr\u00fcn \u00f6nemini g\u00f6stermektedir. Ayr\u0131ca servikal b\u00f6lge i\u00e7in yap\u0131lan bir \u00e7al\u0131\u015fmada disk hernisi g\u00f6r\u00fclme s\u0131kl\u0131\u011f\u0131n\u0131n u\u00e7ucularda normal pop\u00fclasyona g\u00f6re biraz daha fazla olmakla birlikte, bu fazlal\u0131\u011f\u0131n istatistiksel anlaml\u0131 olmad\u0131\u011f\u0131 g\u00f6sterilmi\u015ftir.<\/p>\n<p style=\"text-align: justify;\">Helikopter u\u00e7ucular\u0131nda ise vibrasyona ba\u011fl\u0131 olarak omurgan\u0131n iki yan\u0131nda uzanan kaslar\u0131n yorgunlu\u011funun a\u011fr\u0131lara neden olaca\u011f\u0131 d\u00fc\u015f\u00fcncesiyle, u\u00e7u\u015f esnas\u0131nda EMG \u00e7ekilerek yap\u0131lan \u00e7al\u0131\u015fmalarda bu g\u00f6r\u00fc\u015f\u00fc destekler bulgu saptanmam\u0131\u015ft\u0131r. \u00c7al\u0131\u015fmalar\u0131n t\u00fcm\u00fcnde a\u011fr\u0131 sebebi olarak helikopter u\u00e7ucular\u0131n\u0131n u\u00e7u\u015f esnas\u0131ndaki post\u00fcr\u00fcn\u00fcn sorumlu oldu\u011fu bildirilmi\u015ftir.
